Usernames and passwords are case sensitive.
From the log in page, click on the link for “Forgot your username?” Or “Forgot your password?” This will open a page with your challenge questions allowing you to recover your username or password.
(Don’t request another activation link.)

Don’t have an account yet?
New patients will receive an activation link in an email sent by our registration staff after completing new Patient Registration Forms and checking in for first appointments. From a computer, tablet or phone with internet, this link will enable you to log in and create your own username and password. Your unique link will expire after 7 days.
Established patients who do not have a Patient Portal account, can alert our registration staff at your next clinic visit and they will assist you in signing up for the Patient Portal. Or contact Technical Support.
