If Your Insurance Is Not Listed

We strive to keep our insurance list as current and comprehensive as possible. If you do not see your insurance plan listed, please call our office to speak with one of our insurance specialists. We will be happy to verify whether your plan is accepted.

Billing Questions & Payment Information

Insurance Filing

We will file insurance claims on your behalf. Due to frequent changes in insurance requirements, a copy of your current insurance card is required. When claims are submitted, we request that payment be sent directly to our office.

Co-Payments

Many insurance plans require a co-payment at the time of service. Please be prepared to pay your co-payment when you arrive for your appointment.

Deductibles

Some plans require patients to meet a deductible. If applicable, payment toward your deductible may be due at the time of your visit.
